Demodex is a parasite that causes a disease called demodicosis (also known as demodectic mange). It is very common because most adults carry Demodex mites. However, due to mild symptoms, demodicosis is often mistaken for various skin, eye diseases, or allergic reactions.

Demodex is a small arachnid that causes the disease called demodicosis. Demodex feeds in hair follicles and sebaceous glands, nourishing itself on lipids and skin sebum. The highest concentration of Demodex mites is found around the nose, around the eyes, on the forehead, chin, and nasolabial fold. They can also be found on other parts of the body, e.g., on the hands or feet, on the scalp, eyebrows, eyelashes, as well as in intimate areas.

Infection can occur through direct contact with an infected person, touching the same objects as the infected person (e.g., using their clothing, comb, or even cosmetics such as testers in drugstores). The risk of infection is especially increased when using services of beauticians and hairdressers.

Demodex eggs are found in dust and are also airborne.

 

Demodex (demodicosis) - symptoms of infection

In some people, demodicosis may be asymptomatic. In others, it causes various skin symptoms such as:

redness
scaling
rash
itching
papules and pustules resembling acne
etc.

Demodex can also contribute to the development of blackheads and comedones. It may accompany common and rosacea acne, angular stomatitis, seborrheic dermatitis, etc., usually worsening the symptoms of these diseases.

Sometimes hair loss occurs in the person affected by Demodex, which is mistaken for alopecia areata.

If the parasite feeds near the eye, it can lead to inflammation of the eyelid margins, dry eye syndrome, and allergic inflammations of the eye surface. Symptoms then include:

persistent itching
burning
redness and swelling of the eyelids (often visible dilated blood vessels)

 

Demodex is mainly active at night, so symptoms may worsen during this time:

dry eyes and sensation of sand under the eyelids,
deposits, scales on the eyelid margins,
deposits around the base of eyelashes (cylindrical dandruff),
discoloration of eyelashes, which are brittle and fall out very easily,
photosensitivity,
dust or smoke,
bacterial superinfection, manifested by stye or chalazion,
fungal infection
